I understand there are certain risks involved participating in physical training, including potential personal injury.

 

I acknowledge that The Player Project will do everything in their power to create a safe environment and look out for the well-being of myself and/or my child during any training. I acknowledge that The Player Project are not responsible for any injury that might occur.

 

I understand that The Player Project does not provide insurance to cover expenses for participants and it is strongly recommended that I carry my own medical insurance.

 

I fully release The Player Project, their employees, officers, and agents from all liability in connection with my participation in training.

Permission to Use Image

 

I grant permission to The Player Project to use photographs and/or video of me and/or my child taken during participation of any training.

 

I understand images may be used in publications, news releases, online, and in other communications related to The Player Project.
